ELDER IN-HOME CARE

• Medical assessments, hospital discharge assessments, home maker services, personal care aid services.
• Life Line – assistance with set up
• Liasion with Interior Health Services and care
• Set and assessments for Long term care sites
• Transport and support with physician appointments
• Medical supplies, aides available for on-loan as needed

Currently – 3 Band members – Home Making staff support workers and 1 Certified Personal Care Aide  band member staff

Care for Elders and persons with disabilities
Diabetes – couselling and resourses available thorugh other health staff team members.
